Of course, one thing you can do is is pick up a virus running strange random binaries so thats about the only thing you should be careful about.

If youre unsure you can try uploading the file to virustotal before running it yourself.

Maybe a setting or two might be outside of your monitors range.

Setting the resolution too low might make the game hard to play.

But most of the changes are painless and temporary.

Changing settings within the game files can be bad for the game, if you dont make copies before you change them.

Those game file settings to be changed to unplayable values.

Changing the windows registry is a disaster waiting to happen, if you arent prepared to deal with the consequences to your operating system.

Ill just file this under NOT WORTH IT.

Downloading random programs from unknown websites is generally a bad idea, from a security standpoint.

Personally, I only download mods from specific verifiable websites, and only for specific games.

And even then, most games arent worth messing with, beyond the Options Menu.
